Objectives
The Post-COVID in Primary Health Care Practice Supports is an initiative of Primary Health Care and Chronic Disease Management Network, in partnership with the COVID Network at Nova Scotia Health, clinicians, patients and partners, aimed at supporting primary care providers and clinicians in the treatment and management of patients with post-COVID symptoms in primary care settings.
Audience
The goal of this initiative is to build capacity in primary health care providers and teams to support their patients who continue to experience symptoms related to COVID-19 for a longer period of time, following the infectious phase of the disease, building on evidence emerging from other jurisdictions.
