The Care by Design program ensures residents in long-term care facilities receive high-quality and consistent primary care. As a Care by Design physician or nurse practitioner, you will collaborate with a dedicated team of family physicians and nurse practitioners to enhance continuity and quality of care, focusing on medication safety, reducing emergency transfers, and empowering residents in their care planning.

Funded and led by Nova Scotia Health (NSH) Primary Care, the program features a collaborative leadership structure involving Medical Directors and facility teams to develop improvement policies that are developed jointly and implemented consistently across all facilities.

Care by Design Conference
