Goal of Care discussions are intended to achieve a shared understanding between a health care provider and patient or substitute decision maker, chosen family and caregivers. These discussions focus on the patient’s overall priorities and expectations for their health care.
The aim is to support shared decision-making about what medical interventions a patient would want when their health declines. Preferences and decisions are made in consideration of the best available medical evidence, provider experience, and patient priorities.
Nova Scotia Health now has a standardized form that health care providers should use in documentation of goals of care and chosen level of intervention. A policy and care directive outline roles, responsibilities, and expectations of the health care team who are involved in these discussions. Additionally, a range of practice support resources have been developed for health care providers as well as patients/SDMs, chosen families, and caregivers to raise awareness of the importance of, and support effective goals of care discussions.
