Here you will find an inventory of available digital health forms. Most of these forms are available in your EMR software, however, they are made available here for providers without EMR access.
IMPORTANT NOTE: To ensure you are using the latest version of the forms, please access the updated versions in your EMR (sorting by date) rather than using a previously downloaded copy. Alternatively, your can download the most up to date version from this site each time before use. This will help prevent the accidental use of outdated forms.

Nova Scotia Health Authority's Information Management & Technology Primary Health Care and Community Applications team (PHCCA) will assist eligible health care providers in completing the process to establish an electronic results delivery to their practice (e.g. lab and diagnostic imaging).
Requesting the delivery of Laboratory and Diagnostic Imaging Results for a Provider can take up to a month to complete. Providing the PHCCA team with as much advanced notice as possible and responding in a timely manner will ensure that you are added as quickly as possible. To start your e-results setup, please click the following link: Set Up Access to Electronic Results and follow the steps provided.
