DESCRIPTION:The Behaviour Change Counselling Development Program (BCCDP) 
 accelerated pathway is opened only to primary health care staff. It provid
 es education and skills training in supporting patients to develop self-ma
 nagement skills to effectively activate and sustain health behaviour chang
 es. It is offered by the Primary Health Care &amp\; Chronic Disease Manage
 ment Provincial Network. \n\nWhy Take This Program? \n\n\n\nProvides pract
 ical\, evidence-based tools to support behaviour change where it’s most 
 possible. \n\n\n\n\n\nFosters effective motivational communication skills.
  \n\n\n\n\n\nSupports Nova Scotia’s Chronic Disease Management and Welln
 ess Strategy by promoting self-management. \n\n\n\n\n\nEmpowers patients a
 s partners\, improving care quality and sustainability. \n\n\n\n\n\nRespec
 ts the well-being of both patients and providers. \n\n\n\n\n\nPrioritizes 
 prevention\, wellness\, and self-management for an aging population with r
 ising chronic conditions. \n\n\n\nThe program includes eight self-directed
  modules and three live practice workshops. Workshop 1 runs monthly beginn
 ing September 24th. If you want to complete the whole program it runs from
