DESCRIPTION:This 3-credit-per-hour group and self-learning assessment progr
 am has been certified by the College of Family Physicians of Canada. Earn 
 up to 42 certified Mainpro+® credits! All modules and workshops are manda
 tory to be eligible for Mainpro+ credits. \n\nThe Behaviour Change Counsel
 ling Development Program (BCCDP) provides education\, skills training an
 d support for Family Physicians\, Nurse Practitioners\, Medical Residents 
 and Medical Students who are interested in supporting patients to develop 
 self-management skills to effectively activate and sustain health behaviou
 r changes. It is offered by the Primary Health Care &amp\; Chronic Disease
  Management Provincial Network and was the recipient of the Patient and Fa
 mily choice Quality Award for 2024. \n\n \n\nBenefits to Physicians and Nu
 rse Practitioners \n\n\n\n90% of providers would recommend the program to 
 a colleague. \n\n\n\n\n\nAll providers reported increased knowledge\, skil
 l\, and confidence in using behaviour change counselling skills and nearly
  all have changed their practice accordingly. \n\n\n\n\n\nCompetency impro
 ved as a result of the program\, as measured by the average pre-test and p
