Spine Assessment Clinic - Referral Form
- The Spine Assessment Clinic is part of Nova Scotia’s electronic referral tool (Ocean eReferrals).
- Do not use this form if you are set up to send eReferrals.
- Send your referral to the Spine Assessment Clinic listing using the eReferrals tool.
- Please encourage your patient to provide their email address. This will allow them to receive automatic notifications on their referral.
- Effective April 1, 2024, use of eReferrals is mandatory for primary care providers / surgeons making surgical, MRIs and ultrasound referrals.
- Primary care providers and surgeons who are not yet set up to use eReferrals, or who are not currently in scope to use the tool (i.e., emergency physicians), should send referrals using this new standardized Spine Assessment Clinic referral form.
- Your referral will flow to the Spine Assessment Clinic for follow-up and will be manually entered in Ocean eReferrals.
- Please encourage your patient to provide their email address. This will allow them to receive automatic notifications on their referral.
