Practice Facilitation: Helping Primary Health Care Teams Improve Workflow, Access, and Patient Care
Primary health care teams often face challenges that can make work harder and days longer than expected. Practice Facilitators work alongside primary health care teams and clinics to help make day-to-day work smoother, more efficient and better aligned with what patients and providers need.
What is a Practice Facilitator?
A Practice Facilitator is a Primary Health Care Coordinator who works directly with clinic teams to help them improve how care is delivered. Practice Facilitators are trained in quality improvement and change management, and they use those skills to support clinics in strengthening workflows, using data effectively, and building strong, collaborative care teams.
They don’t direct, mandate changes or regularly take over ongoing operational tasks. Instead, they act as coaches and partners, helping teams identify opportunities, test new approaches, and build confidence and capacity to lead ongoing improvement on their own.
Practice Facilitators operate across all Zones and Primary Health Care governance models. While structures may vary across jurisdictions, their core purpose remains consistent: to build internal capacity within teams and enable them to lead and sustain improvement.
Their work is grounded in three main areas of focus:
- Supporting teams to identify improvement priorities, determine goals, develop strategies and drive progress.
- Helping clinic teams strengthen how they work based on proven primary health care best practices, through coaching and practical, hands-on support.
- Turning provincial or zone priorities into real, day-to-day action at the clinic level.
In short, Practice Facilitators help clinics make meaningful, sustainable changes that improve care for patients and support a high-functioning primary health care system.
Common Ways that Practice Facilitators Help Teams
Practice Facilitators partner with providers and clinic teams to strengthen clinic operations and support high‑quality, sustainable primary care. They use data‑driven insights, structured improvement methods, and practical tools your team can apply right away.
Using the Nova Scotia Health Quality Improvement Approach, Practice Facilitators help teams:
- Improve clinic flow and overall efficiency
- Enhance access to care and continuity
- Strengthen communication and clarify team roles
- Use EMRs and digital tools more effectively
- Support proactive panel management
- Analyze data to identify opportunities and measure progress
- Navigate and connect teams to other supports and resources
Support is hands‑on and tailored to the realities of the clinic. Practice Facilitators coach teams through practical tests of change and help integrate successful improvements into everyday practice. While they do not usually provide conflict management, IT support, or day‑to‑day clinic work, they instead focus on optimizing workflows and building long‑term efficiency within teams.

Why This Work Matters
Evidence shows that clinics supported by Practice Facilitators are up to three times more likely to implement evidence-based improvements and see better clinical outcomes, stronger teamwork and reduced administrative burden. Practice Facilitators help create conditions for high-functioning teams, better patient flow, and sustainable change so that providers can spend more time on patient care and less time on process challenges.
