The Integrated Access and Flow Network (A&F Network) is pleased to present “ 365 Days in Review” - a look back at the past year’s initiatives, programs, progress, learnings and achievements. The A&F Network’s work is only possible with the engagement of zone leaders, collaborators and all partners, especially the patient family advisors, who generously and freely give their time.
The A&F Network enables change to improve patient-centered care and streamline patient flow across the health and social care systems. It emphasizes timely access and appropriate treatment to enhance the overall patient experience and recovery journey. Applying an interdisciplinary practice and strategic collaboration with other networks and programs, the A&F Network ensures a provincial approach to aligning services, initiatives and planning across all zones and provincial programs to maintain consistency, coordination, quality, efficiency and system sustainability.
